"Need urgent fit note"

Dear Doctor,

My name is Linda George and my date of birth is 10 July 1960. I am writing to let you know that I am very disappointed with the length of time that I have to wait to see or speak with a doctor for a fit note. I having been calling and only to be told I cannot see or speak to a doctor to obtain a sick note. I came in personally to the surgery today having been on the telephone queue of over number 30 of the doctors queuing system. When I came to the surgery was literally empty and there's no one as far as I can see waiting to see a doctor. Please I need an urgent fit note for my mental health and Arthritis which has worsened and I am unable to look for work and want to be assessed for limited capability and a fit note is an essential part of the health assessment that is needed. Please could someone help me with an earlier appointment or issue the fit note because my condition is well known documented.
