About: South West & Southampton Division / Frailty Support Team - Totton and Waterside

Frailty team Totton

The team were asked to provide outreach to my dad who is very frail and has dementia with challenges around communication.

Real effort was made to communicate with dad and used his chosen name (not his name on medical records but his nickname). This may sound very basic but really important. An advanced careplan was drawn up to help us make dads wishes known and stay at home should he become suddenly unwell.

I really feel that a significant amount of time and effort was made in getting to know dad in the short time he was supported by the team before recovering.

We felt very supported by the team and can only thank them for all their time and effort with dad. Seems inadequate but they made a real difference to us.
